If you are submitting an HSA change request, please use our new e-form in PowerSchool Employee Records.

Use link: PowerSchool Employee Records

  • Log-in with your PHASD username and password

  • Go to "Available Forms" and choose "HSA Election"

  • To finalize your form, choose SAVE FINAL in the bottom right corner to enable the e-signature.

  • The form will re-load for you to sign and submit.
