Many of our Staff Forms are now on Employee Records:

  • Log-in with your Port Huron Area School District email and password.

  • Go to available forms on the left panel and choose the form you need.

  • Choose add for myself and click add as an e-form.

  • Once completed, choose save final to enable the e-signature and then submit the form.
